Players being kicked from games

I have seen it in almost every game iv played up to now. I have the english version of the game. What happens is, the game just says "[SYSTEM](player name) has been kicked out of the game". Now could you explain to me what happens here? Doe sthis message show up when a player quits the game or is it when the game actually kicks the player out it's self?

When does it happen? At launch of the game? Is the host or some of the players clicking on the person's button in the waiting window?

It happens mostly when we are in a game are playing normally. Sometimes near the start, sometimes near the end, but always once the game has actually started. The reason i dont understamd it is because sometimes it has happened to an ally on my team right in the middle of an attackagainst the enemy, so there would be no reason for them to quit.
