Can't find CD key
The CD key is located in the jewel case under the third CD (US versions) or at the back of the manual (apparently some international versions).
If the white label is blank, it was mis-printed. Return to the store, show them the problem, and ask for a new copy. Make sure to open that copy in store to ensure that there is a CD key included.
Lost CD key
The number for THQ customer support is 818-880-0456 and can be reached Monday-Friday, 9AM - 5PM PST. In order to receive a new CD-Key we basically need to just exchange your copy of the game for a new one, which means it must be within 90 days of your place of purchase. Otherwise the exchange is an out of warranty exchange as detailed on the last page of the manual for the game (meaning there is a replacement fee of $15 to replace the game)
