Custom Avatar?

How do you add your own custom avatar (image on the left, under your name) rather than just from choosing one on the list?

You don't. Custom Avatars are usually awarded to people who have done exceptional things here on the boards or in the community to make themselves stand out. I believe this question would have been better in the forum issues section.
